The pursuit of facial harmony is a highly personalized journey that increasingly involves male patients. A very common issue, especially among boys and young men, is the presence of prominent ears. While this anatomical trait is common in both sexes, women can frequently disguise the issue with their hair length and styling. For men, however, this feature is often fully exposed and can easily become a source of profound psychological discomfort and social insecurity.
When a physical trait becomes a burden, plastic surgery offers definitive, natural-looking solutions. In this specific clinical case, we opted for a comprehensive approach. We performed a combined procedure to address not only the prominent ears but also a significant disharmony in the nasal profile, ensuring a balanced final result.
Correcting Prominent Ears (Otoplasty)
The primary anatomical issue with the patient’s ears was the absence of a physiological fold known as the antihelix. Through precise surgical intervention, we meticulously recreated this natural fold. This fundamental step allowed us to correct the ear’s orientation, effectively pinning it back. The result is an ear that is perfectly aligned with the head, no longer prominent, and integrates discreetly and harmoniously with the rest of the facial features.
The Challenge of Rhinoplasty with Thick Skin
Simultaneously, we tackled a complex rhinoplasty. The patient’s nose presented several aesthetic challenges: a rounded and drooping tip, a prominent dorsal hump, and, most importantly, very thick skin. Thick skin poses a unique challenge for the surgeon because it tends to obscure the refined details of the underlying nasal structure. Furthermore, the skin itself cannot be surgically thinned out without risking severe vascular or scarring complications.
To overcome this limitation, we focused intensely on reshaping the underlying bone and cartilage framework. We removed the excess curvature by eliminating the dorsal hump. Next, we rotated the tip upward to provide a more graceful, elegant appearance, and sutured the two lower lateral cartilages together to narrow and define the tip. By building a strong, sharply defined structural foundation beneath the tissues, we achieved a harmonious and well-proportioned nose that looks great despite the inherent limitations of thick skin.
